A municipal corporation in Lisbon enrolled and launched a chronic disease monitoring program for geriatric residents under that municipal area. Monitoring of hypertension, blood glucose and ECG started using CareTechr’s Care Ring and Care V6 devices.

CareTechr’s Platform continuously tracked citizen vitals, detecting early risk signals. One fine day a 68-year-old citizen living alone showed  abnormalities in HR, HRV and no activity for extended hours—showing early signs of a potential fall or cardiac problem.

AI assessed the situation generating an orange alert which immediately trigger an agentic AI check-up call but when failed in getting response after 3 failed attempts, an escalation was made to family member along with alert notification to medical care team for timely action and immediate ambulance dispatch.

This enabled faster response times, timely ambulance dispatch, and early ER access, ensuring optimal use of the golden minute and golden hour. As a result, complications were reduced and emergency hospitalization helped in reducing fatality.

Outcome: Improved population-level chronic disease management with quicker interventions and reduced risk of severe complications. Citizen’s trust on municipality increased, Municipality also can position itself as citizen friendly.
